Probably will never know for certain but it might have been an injury that got infected.

So many things it could be. What is the pH? Water temp? How about dissolved oxygen? Any other water parameters tested like ammonia, nitrite and nitrate? Did you test the salt level?

The pH can drop rapidly on some systems, as well as pumping problems, higher temps or algae creating an oxygen problem. Did something die and overload your system with ammonia/nitrite?

Look at the big picture, make sure the water is optimum for the fish / environment, then your fish have the best chance for survival.

My brother had a growbed he introduced into his system that he did not scrub thoroughly. Several months later his fish started dying around 1 per week. After chasing many possible causes he emptied his growbeds, properly cleaned the containers, and all is well ever since.

I had a problem with ammonia and nitrite once and it started several weeks after I put my pumps on a timer to shut off at night. Once I went back to 24 hr pumps all went back to normal (thanks Scotty!).

I've had a few disease outbreaks and salting has always kept them in check. I use a mixture of potassium chloride and sodium chloride and make sure they are pure salt, no additives for anti-caking or iodine, etc.
